Now let's talk a little about emphysema. So emphysema can be seen with Marfan syndrome and what it is, is the loss of alveolar walls and generally this can be developmental or genetic or it can be destructive.

And the first case, this is what we see with Marfan syndrome where it seems to be developmental and genetic but in the case of persons who are smokers is what we call destructive and the manifestation of this is that there's a large enlargement of air spaces and this leads to airway obstruction as well as inflammation as well as symptoms.

And on this slide, we see a schematic of exactly what emphysema looks like. On the right bottom, we see what I call the BlackBerry structure that's emblematic of a normal kind of alveolar structure where you get lots of these tiny spheres where you have a thin wall. An oxygen enters the lung, enters the alveoli and goes into the wall where we have the blood vessels and that's how we oxygenate all of the critical organs in our body.

But in the setting of emphysema, there's either the lack of a formation of all of these walls or destruction of these walls and therefore were unable to move oxygen very efficiently in the bloodstream and the lung function is compromised and that is why persons with emphysema develop shortness of breath and develop cough and develop respiratory symptoms.
