So with that let's talk a little bit about aortic arch surgery specifically. So there's several different types of arch procedures. I just wanted to make it simpler for everyone so that you could retain this information and we're going to look at three kind of common procedures.

The first is the hemiarch. The hemiarch is really where you're doing a partial arch and this is what we call a peninsula style technique where you essentially have a peninsula of dacron going underneath the arch. The purpose here is to try and eliminate as much arch tissue without doing a total arch and in the acute settings such as an aortic dissection this is found to do very well for connective tissue disorder patients as well as non-connective tissue disorder patients and by far is the most commonest procedure that we do.

まずはじめはヘミアーチ法(部分弓部置換術:左の図)です。この方法は弓部の一部分を置換します。人工血管が弓部の下に入り込んでいることから、半島状再建とも呼ばれています。弓部全置換を行わないまでも、できる限り弓部組織を少なくしたいときに行われます。大動脈解離などの急性大動脈疾患の場合、結合組織疾患の有無を問わず、非常に有効な方法であることが分かっています。施術回数が断トツに多い方法です。

 

Next is the island reimplant. Why is it called an island? Well we're taking an island of the head vessels and reimplanting it into the graft and then suturing it into the proximal descending thoracic aorta. Okay? This is not what we recommend or would do for most patients with connective tissue disorder, especially Marfan or Loeys-Dietz because we're leaving aortic tissue behind.

次は島状再建(真ん中の図)です。なぜこのような名称かというと、頭部に向かう血管を島状に切除し、人工血管に縫い付け、人工血管の端を近位下行動脈と吻合するからです。この方法は大部分の結合組織疾患の患者さん、特にMarfan症候群やLoeys-Dietz症候群の患者さんには推奨していません。なぜかというと、弓部組織が残ってしまうからです。

 

So the most commonest procedure is where we do a total reimplantation. Total reimplantation means we're going to get rid of all the aortic tissue and we're going to do individual reimplantations.Now this picture is a representation of what we call a four branch arch technique. Four being the three to the head vessels and then the fourth is a perfusion graft so we can provide blood flow from the heart lung machine. There's a variety of configurations that surgeons have done to reconstruct these vessels and there's a variety of commercially available grafts that allow us to make these connections.

One is there's trifurcated grafts where you connect the head vessels together and then you reimplant it into the arch in one location, so not to bore you with the technical aspects of it we have lots of tools in our tool belt.

例えば、この図のような三叉に分かれた人工血管では、頭部への血管と吻合し、それを弓部のどこかに縫い付けることができます。技術的なことについては深入りはしませんが、使える道具は色々あるということです。
