Q:
The next question and I think this is this could probably be taken a little more broadly even. My 22-year-old son was diagnosed with LDS4 one year ago. The only thing he's done is the head to pelvis MRAs. He's in denial since he doesn't exhibit any symptoms, pain, pains or problems. I'm trying to nudge him to speak to a medical team to monitor his condition. What specialist should he see first?

You know, so you're not alone in facing the problem. Problematic people tend to be relatively young and as Angela was mentioning have this view of invincibility. I would honestly say that men are typically more problematic than women that might be a generalizable rule.

But I think it's important to get the message across that we've learned a lot of lessons about how to care for people with Loeys-Dietz syndrome and we think what we're doing makes a difference. In fact in many respects we're sure that what we're doing makes a difference including, for example, referring for surgery at smaller aortic dimensions.

A message that I'd want to get across to a person who's sort of in denial is that many people with a wide variety of vascular connective tissue disorders feel just fine. They feel normal. They feel perfect until the moment a blood vessel ruptures that, you know, that could, for example, lead to their death so, you know, while feeling good is great it should not be taken as, you know, complete reassurance that there is no problem brewing.

I would certainly recommend that this young man sees, identifies someone that is knowledgeable about the diagnosis. That might be a cardiologist. It might be a geneticist. In some circumstances there are general practitioners who have learned quite a bit about the condition. 

質問者さんの息子さんには、循環器内科医、遺伝専門医など、LDSについての専門家に診てもらうことを勧めます。状況によっては、LDSに関して相当の知識をお持ちの一般開業医でも構いません。

 

In any event he should find someone that's either knowledgeable or curious and willing to learn that's going to help manage his care. I do think that seeing a cardiologist or someone who's going to review cardiovascular imaging studies on a regular basis is important. You know, again someone who's familiar with the use of medications, the interpretation of genetic testing would also be very helpful. 

いずれにせよ、LDSの専門知識があったり、LDSに関心をもっていたり、LDSのことを学ぶ意欲のある医療者を見つけ、管理をお願いすることです。循環器内科医など、定期的に心血管系の画像診断ができる医療者を受診することが大切です。また、薬の使い方を熟知していたり、遺伝子検査結果の解釈に経験を積んだ医療者も、大いに助けとなるでしょう。

 

Some people don't need to see an orthopedist frequently and others do because they have significant bone problems. 

骨格系に重篤な所見のある患者さんでは、頻繁に整形外科にかかる必要がありますが、そうでない方もいらっしゃいます。

 

Unlike Marfan syndrome eye problems in Loeys-Dietz syndrome are relatively uncommon so, you know, an ophthalmologist, while everyone should see one intermittently, is not necessarily someone that should be seen on a regular basis or a frequent basis in someone with Loeys-Dietz syndrome. 

MFSとは異なり、LDSでは眼疾患は比較的稀です。一般の方でも時々眼科を受診したほうがいいのですが、LDS患者さんの場合には定期的あるいは頻繁にかかる必要はありません。

 

I really think it's a combination of a primary medical doctor, a cardiologist and/or a geneticist that's going to be able to provide the most value.
