I don't have any formal disclosures to declare. We will be discussing off-label use of medication. What that means is we'll be discussing FDA approved medications that we commonly use in pediatric patients but not for their original intended purpose and the use of medicines for aortic dilatation is an off-label fashion in this way, particularly in pediatrics. The only other thing I do have to disclose is this is my twin brother and he works for a drug company and unfortunately they put him all over the website and this is true. I've been told sometimes when I show this that people don't believe it but this really is true and he's in Indianapolis so if you come across him and say Hi and he doesn't recognize you or something that's not me but all of that is true.

One of the things that I was really looking for in 2020 like most everyone else was good news and this was probably the greatest news I saw in 2020. This was a review of the incidents of cardiovascular events in children with Marfan syndrome, kind of during a longitudinal study and this was done by the group in Europe and it was a really good study because it was fairly large for a pediatric study.

They looked at 462 patients with a median follow-up of 17 years and the majority of them were in their second decade of life at age of completion of the study and during that time there was only about a five percent risk of aortic events and the majority of those actually were prophylactic aortic root surgery for patients who had reached dimensions where intervention was warranted and there were only two total cases of dissection out of the 462 patients and one of them actually was secondary to a motor vehicle accident. So, you know, essentially in a group of patients being followed and under appropriate medical care out of 462 there was only one that had an unprompted aortic dissection, which is just really encouraging news for us to be able to share kind of with families particularly at the time of new diagnosis when there's really a lot of information to know.

Additionally they were able to identify that the group of patients at the greatest risk for a childhood event all shared kind of a larger aortic z-score. So greater than size three before age 16, more rapid aortic growth five mil, five millimeters or more in a year or an aortic z-score that kind of increased over time as well and those are. These are all things that intuitively we would expect. If your aorta is more severely dilated you're at greater risk. If it's growing more rapidly we would also suspect you're at greater risk but that really helps us to kind of better frame what an individual patient's risk may be.
