Now what about aneurysms in other locations and what about the timing of surgery? 

動脈瘤ができる場所と手術タイミングについての質問です。

 

This is a complicated slide and but just the main point here is this. So this yellow area indicates changes of what they call cystic medial necrosis, which the pathologists have actually changed this term, they don't use it anymore but here they used it and these are the kinds of changes you see in Marfan syndrome in the wall of the aorta and these changes are very prominent in the, in the beginning of the aorta and the root and far less prominent so the yellow stuff is much less as you go down the aorta so the pathology of the aorta changes as you go down.

こちらのスライドは、ゴチャゴチャしていてわかりにくいと思いますが、(棒グラフの)黄色い部分はMarfan患者の大動脈壁で生じた嚢胞性中膜壊死(CMN)の割合を示しています。ちなみに、この用語は病理医が変更しましたので、もう使われていません。大動脈起始部や大動脈基部では、CMNの割合が顕著に高くなっていますが、下行大動脈でのCMNの割合はより小さくなっています。つまり、大動脈の病理は大動脈の先に進むにつれ、変化することがわかります。

 

And this is another complicated slide but just to briefly show you what it is these are. Embryological origins of cells and tissues in the periphery and they're color-coded. So you can see that the very beginning of the aorta comes from an embryological structure called the secondary heart field whereas the ascending aorta comes from the embryological structure called the neural crest and so on so the, the cells that populate the wall of the aorta are not the same as you go down the aorta. They really come from different embryological origins since this is one of the reasons why different parts of the aorta are affected in different ways. 

こちらのスライドもわかりにくいと思いますが、各細胞とその周辺組織を発生学上の起源に基づいて色分けした図です。この図を見ると、大動脈の最も心臓に近い部分は、二次心臓領域と呼ばれる発生学上の構造に、上行大動脈は神経堤と呼ばれる発生学上の構造に由来することがわかります。つまり、大動脈壁を構成する細胞は同一ではないということです。このため、大動脈各部の影響の受け方には違いが生じることになるのです。

 

Clinically, the pattern we see is sometimes, it's the ascending aorta but the rest is okay. Here's a patient. It's actually a vEDS patient, who has very minimal aneurysms and yet they're still clinically important. This is a composite of 30 vEDS patients in, in a Mayo series and this is where all these 30 patients had aneurysms in these various places. Also as Dr. Lannucci mentioned there can be other signs like tortuosity. 

臨床上の動脈瘤のでき方としては、このように大動脈基部のみが膨らみ、その他の部位に変化のないパターンがあります。こちらはvEDSの患者さんの血管です。基部の膨らみは非常に小さいのですが、臨床上はやはり重要です。30名のvEDSの患者さんの動脈瘤を合成したものがこちらになります。様々な部位にできているのがわかります。また、先程Lannucci先生からあったように動脈のねじれも見られます。

 

And the when to operate? Well, as far as the Marfan syndrome goes and when it's the aortic root Dr. Jondeau's group in France showed that the risk of dissection per year is quite low at these smaller millimeters but getting a little bit increasing in 45 to 49 and significantly increasing after 50, so this is why the general recommendation is to have aortic replacement between 45 and 50 if it's Marfan syndrome. 

手術タイミングについてです。Marfan症候群の大動脈基部に関しては、フランスのJondeau医師のグループによるこちらの研究があります。大動脈径が小さい場合の年間の大動脈解離発症リスクは非常に低いのですが、45~49mmあたりから徐々に高まり、50mmを超えるとかなりのリスクになります。そのため、Marfan症候群においては、大動脈基部が45~50mmが一般的な手術適応サイズとなります。

 

Now I'm just going to not say too much about Turner. It's kind of a special case but in Loeys-Dietz syndrome, which is a more dangerous condition, this is when you operate. Marfan, I said of 45 to 50 and then bicuspid valves are safer so you can wait longer. 

Turner症候群については特別なケースということもあり、深入りしません。Marfan症候群よりリスクの高いLoeys-Dietz症候群では、このくらい(4.0cm超)が目安です。大動脈二尖弁はそれほどリスクは高くありませんので、手術タイミングを遅らせることができます。

 

But all this changes if there's a certain risk factors. For example, family history of dissection, personal history of dissection in another location or if there's rapid enlargement or if there's a planned pregnancy. So these are you kind of have to work this out with your a specialist taking care of you now. 

ですが、特定のリスク因子、例えば、大動脈解離の家族歴、患者さん自身の大動脈解離の既往、拡張速度が速い、妊娠の予定があるなどの場合、これら目安は変わります。こうしたケースでは、主治医の先生との相談が必要となります。
