Exercise and Marfan, LDS, and VEDS: The Latest Research

This is kind of an overview of what we're going to share tonight and we'll try to get through. There's a lot of material, so we'll cover some of it more quickly and try to spend more time at the end.

But basically what we want to do is set the background for what do we know about exercise and genetic aortic conditions, give some information about the clinical aspect of exercise and recreational exercise, in particular Dr. Prakash is going to review what's known about the research based on mouse models and some of the clinical research that that he has pioneered in this area to help understand what can be considered safe and low risk, then we're going to just highlight of six or seven of the abstracts presented at the science in Paris International Symposium from just very recently. These haven't made it into a publication yet but they're very important abstracts emphasizing that the problem in the condition and then potential solutions with exercise and then talking little bit about aortic dissection, exercise, quality of life and then end with information about an upcoming clinical trial. 

 

大まかな流れとして、まず、運動と遺伝性大動脈疾患についてわかっていることを理解していただくために必要なバックグラウンドを説明し、次に、運動および娯楽的スポーツの臨床的な側面についての情報を提供します。Prakash先生からは、特にマウスモデルによる研究からわかったこと、および先生ご自身が先駆けとなっておられる臨床研究において、安全とされる運動やローリスクと考えられる運動について説明いただきます。その後で、直近にパリで開かれた研究集会で発表された6~7例の研究を取り上げて説明します。これらの研究はまだ公開とはとなってはいませんが、遺伝性大動脈疾患における問題や、それらに対する運動による対応策に焦点を当てた重要な抄録となっています。終盤では、大動脈解離と運動、QOLについて、そして、控えている臨床試験について最後に説明します。

 

Individuals who have heritable conditions whether syndromic like Marfan, Loeys-Dietz or vEDS or nonsyndromic, related to a known gene or other or at risk for an aneurysm and of course aortic dissection, so in living life successfully the guiding principle regarding physical activity for individuals with these conditions would be to avoid the most intense training exercise in athletic competition because of concerns in that level of activity with increased aortic wall stress. 

 

遺伝性大動脈疾患には、マルファン症候群やロイス・ディーツ症候群、血管型エーラス・ダンロス症候群のような症状の現れるものや既知の原因遺伝子により発症したり、動脈瘤のリスクが生じたりするなど症状の出ないものがあり、もちろん、大動脈解離も含まれます。こうした疾患の患者さんが順調な人生を歩むにあたり、運動に関して参考にすべき指針があります。それは、勝敗を競うような、最高に激しいトレーニングのような運動を避けるということです。こうした運動では、大動脈の壁にかかる負荷が上昇するためです。

 

And I think, you know, sharing decisions over a quarter century for patients and families there's really been a pendulum swing away from avoiding the sedentary lifestyle to more a prudent shared decision and exercise in a way that's safe and effective and at low risk, so that's the goal really safe exercise what's, what do we know and what's the data, so that's what we try to do tonight.

And again those with heritable disorders avoiding intense activities, severe contact sports, intense weight training and isometric exercise but recreational activities in a low and moderate aerobic pace we really think are low risk, so that's what we want to talk about.

 

遺伝性大動脈疾患の患者さんでは、激しい運動や接触を伴う激しいスポーツ、激しいウェイトトレーニングや等尺性運動を避け、有酸素状態で行う低強度から中強度の娯楽的な運動が推奨されます。これらの運動はローリスクと考えられています。後ほど詳しく説明します。
