Moving on to vascular Ehlers-Danlos syndrome in, and in vEDS. In imaging the aorta and the branches is very important because it may identify arterial segments at risk that are asymptomatic but the frequency in, in type of screening surveillance is uncertain. In general, in the individual who's asymptomatic with vascular Ehlers-Danlos syndrome protocols typically involve MR or CT angiography from the head to the pelvis to evaluate the entire aorta and the branches even without any symptoms and like Loeys-Dietz syndrome in any abnormal segments and similar to Marfan syndrome if the segment is abnormal, in other words, there's an aneurysm or a dissection imaging frequently at that segment over time is necessary but if the arteries look normal probably imaging every two years as long as remaining asymptomatic is reasonable. 

 

では、血管型エーラス・ダンロス症候群に移ります。大切なのは、大動脈と分岐動脈の画像検査です。無症状でもリスクのある部分を特定できる可能性があるからなのですが、画像検査の頻度については明確になっていません。一般論として、無症状の血管型エーラス・ダンロス症候群の患者さんに関する規定では、症状がない場合であっても、大動脈および分岐動脈全体の評価のために頭部から骨盤までのMRIあるいはCTを行うことになっています。画像検査で異常が見つかった場合、つまり、動脈瘤や解離が見つかった場合は、マルファン症候群やロイス・ディーツ症候群の場合と同様、その箇所の検査頻度を増やす必要がありますが、異常が見当たらない場合には、画像検査は2年おきに行うのが妥当を思われます。

 

One thing we didn't mention is in the guidelines because young people who have a heritable thoracic condition will be imaged over their lifetime. There's a strong emphasis on doing MR angiography over CT angiography for those who could tolerate the claustrophobia and the time and things like that, you don't have any contraindications to do an MR study for surveillance. It's a, it's appropriate and preferred, so when lessons exposure to ionizing radiation over time.

And again the current types of CT scans use very low doses of radiation now, so we certainly use those appropriately but in individuals who can have MR we prefer that sort of routine surveillance over time when the information can be obtained just as accurately, next slide. 

 

CTについてですが、現在の装置が出す放射線量は非常に低く抑えられているので、適切に利用する場合もあるのですが、MRIを受けられる患者さんで、CTと同等の正確な情報が得られるのであれば、通常の定期検査ではMRIの方を優先して受けていただきたいと考えています。

 

Dr. DeAnda, would you comment about, go back, one Eileen, about the surgery and vascular Ehlers-Danlos syndrome.

Yeah, so in vascular Ehlers-Danlos, vEDS, the, the, there is a lot of risk involved in not doing anything and there's a lot of risk in repairing these vessels. I, I, this is something that a lot of surgeons don't like to do. The, the tissue itself is very fragile it's very, it's, it's like wet toilet paper and so there are issues with bleeding complications and, and later on down the road the formation of a pseudo-aneurysms and other issues associated with the procedure but that doesn't mean that you shouldn't, these shouldn't be addressed. 

 

血管型エーラス・ダンロス症候群については、血管の修復手術をするにせよ、しないにせよ、大きなリスクがあり、執刀したがらない外科医は大勢います。組織自体が濡れたトイレットペーパーのように非常にもろく、出血を合併することもあります。術後も動脈瘤に似た膨らみができるなど、手術に起因する問題が生じます。だからといって、手術をしないという理由にはなりません。

 

As noted in the guidelines, rapid arterial aneurysm growth, we listed it as greater than three millimeters per year is considered rapid growth and that would be an indication for intervention but once again this, this will require a multi-disciplinary aortic team including in, in our institution and includes a vascular surgeon and we have recently included a, a pathologist who has an interest in this disease because they can help us understand a little bit more about what, what some of the risks are and can be but there are, there are these are not easy operations and they should be done by some performed by someone who's done more than one of these, next slide.

 

ガイドラインに記載の通り、急激に動脈が拡張した場合は手術適応となります。年間3mmを超える拡張は「急激な拡張」と考えられます。何度もお伝えしているように、手術の場合には、複数の診療科からなる大動脈チームが必要です。私の病院では、チームに血管外科医がおりますが、最近、病理医が加わりました。その先生は、血管型エーラス・ダンロス症候群に興味がある方で、リスクについてのチームの理解が深まると思い、加わっていただきました。簡単な手術ではないことから、血管型エーラス・ダンロス症候群の患者さんを執刀した経験のある外科医が必要です。
