You know, I, when I, when I first started here, you know, as it was asked to kind of focus on aortic surgery we, we were really focused on this was into the early 2000s we were really focused on getting better at taking care of acute problems, you know, saving people with emergencies and we and we still keep getting better at that.

But I noticed that a lot of times the helicopter pad was bringing somebody in who had already survived in acute dissection and, and I don't mean any disrespect to the doctors out there that are taking care of people after we discharged them from having their emergency surgery but I think they weren't quite getting all the attention they needed or had a full understanding of the disease process they were dealing with.

And so we decided that, you know, we need to pay closer attention to these aorta patients with aortic special care and make sure that the aortic people or the surgeons and the teams that are fixing the aorta also monitor the aorta. I know that sounds obvious but it's not the way it's sort of done everywhere although it is now, and we understand the chronic nature of this so that we can stay on top of the disease process but also to educate people.
そこで我々は、特別な大動脈のケアが必要となる患者の皆さんを、以前にもまして注意深く診察することにし、患者さん本人、大動脈手術を執刀した外科医とチームが、確実に大動脈の経過を追うことができるようにしました。当たり前のことだと思うかもしれませんが、現段階でも、全ての医療機関にそうした体制が整っているわけではありません。大動脈疾患は慢性疾患であることがわかっていますので、経過を完全に把握するだけでなく、患者さんへの教育も同時に行っています。
I didn't give them quite the lecture I gave today but when I give them some information about what's going on and I, I literally would see that like the furrow in the brow sort of relax and I always thought this was really amazing how a little bit of knowledge about things can take and assuage fear in people.
患者さんに対しては、今日のようなセミナーを行うわけではなく、ご自身の体が今どういった状態にあるかを伝えています。ほんの少し情報をお伝えしただけで、眉間にシワを寄せた険しい顔が緩んできます。わずかな知識を得ただけで恐怖が和らぐというのは本当に驚くべきことです。
And so and as I've like kind of gone on with this I realized that having fear of of what, you know, you're cruising around thinking your life is totally normal and you're healthy, a lot of, especially, young people because, you know, young people you think they're immortal and then something hits you like this.
このような診察を続けていく中で、突然襲ってくる恐怖というものを理解できるようになりました。何事もなく健康体のまま生きられる、患者さんはそう考えています。特に若者はそうでしょう。自分が死ぬことなど考えられるはずがありません。しかし、大動脈解離のような病気は突然襲ってくるのです。
I ran into a couple patients who were just really like paralyzed by it like, like any time they had a littlest twinge of pain or something like that they thought something bad was happening to them again and and so I, you know, take time to talk to people about it and then we decided that maybe we should study this.
かすかな痛みを感じただけで、体が硬直して動かなくなる患者さんに出会ったことがあります。何か悪いことが起きていると感じているのだと思います。そうした患者さん達と時間をかけて話すうちに、このような方々を対象とした研究を行う必要があると考えるようになりました。
