In Loeys-Dietzs syndrome, we recommend the aorta be operated between
4 and 4.5 cm depending on the specific mutation, the family history, the gender, craniofacial features or other phenotypic features, so somewhere in this range, it's recommended for surgery.

We're learning more about this through large registries that are ongoing such as a Montalcino Aortic Consortium and others around the world, somewhere between 4.5 and 5 cm depending upon the mutation, the family history, the size, the rate of the growth of the aorta, sex of the individual, and others, so these are much less information to be very granular in Loeys Dietz and familial aneurysm syndrome, so there's a little nuance in this, depending upon a lot of factors.

In younger patients with severe craniofacial features and marked phenotypic features of Loeys-Dietz syndrome, 4 cm or greater would be recommended for surgery. If it's picked up later in life and if someone has a TGFßR1 mutation, 4.5 cm would be a very appropriate.
