So a critical problem in the body is how does our blood system age. We are unaware of it but our bone marrow is cranking out gazillions of red cells, white cells, and platelets every day and we just depend on that working normally and if one cell line is altered, for example, if anemia develops or low platelets, it's usually not a serious bone marrow disease, but it's something else in the person's health, but when all three of them began to malfunction, that's usually a bone marrow problem and many of us will get bone marrow malignancies, B-cell lymphomas, Hodgkin's disease, chronic lymphocytic leukemia; these are common malignant disorders.

Now what about our family of patients? They don't seem to have excessive malignancies in the bone marrow, that they do have the eosinophil disorders in the Loeys-Dietz syndrome as I mentioned.

And to my knowledge, this is the only real consequence in the blood system that I know about, but we encounter this in interesting ways. This is a Marfan patient, who is going to have an aortic repair, and just had the regular imaging, but the radiologist noticed the finding here, inside, well that looks like an enlarged lymph node, and so this is a PET scan where the there's a material injected that shows the very metabolically active parts of the body and here, for example, there's this is just a bladder collecting the the radiation, but here is a extremely overactive area metabolically and that the corresponds to what was seen in the other slide; that's a malignant lymph node, so this patient had a lymphoma, so we had to put the aortic repair on pause; she had successful chemotherapy for lymphoma where the lymphoma went away and she had her aortic repair but that would be an example of, you know, finding these unexpectedly because you're dealing with older patients.
これからご紹介する疾患は私が血液系で経験した唯一の重大な疾患です。この画像は大動脈の手術を予定していたマルファン患者さんのものです。いつも行う画像検査だったのですが、放射線科の医師がこの矢印の部分の異常を指摘しました。リンパ節が大きくなっているように見えますね。そこでPETスキャンを行いました。PETスキャンというのは、患者さんに放射性薬剤を注射し、体の中で代謝が活発な部分を見つける検査です。例えば、この下にある赤い部分は膀胱ですが、放射性物質が集まっていることがわかります。上の赤い部分では非常に代謝が活発になっています。この部分は先ほどの矢印の部分と一致しますが、悪性のリンパ節となっていました。つまりこのマルファン患者さんはリンパ腫であることがわかりましたので、大動脈の手術を延期して化学療法を行いました。リンパ腫が消失し治療が成功した後で、大動脈の手術行いました。我々が高齢の患者さんを診ていたことで、偶然このようなケースを経験しました。
