On occasion the descending thoracic aorta is symptomatic and large and has to be replaced first but we know we're going to have to go back and do the ascending aorta so we do what we call a reversed elephant trunk.

We replaced the descending thoracoabdominal aorta here with a Dacron graft but we take the graft and fold it back upon itself and then suture that at the left subclavian artery and then once they've recovered from this operation we come back to median sternotomy and we pull that graft out and now we use to replace the head vessels and the, whatever is necessary with regards to the, the root. We call that a reversed elephant trunk repair.

I don't want to belabor this but in patients that we've done this with, with Marfan syndrome, you can see the mortality rate for the first and second stage are extremely, extremely low.
