Lastly there are special considerations that we have to think about for, for people who have Marfan syndrome and related conditions when we're thinking about heart surgeries.

And generally this is a younger for sure than that if we take all, all patients who come in and need heart surgery. This is much younger group of patients and they may face more surgeries down the road. You have to think about the timing as far as the need for other surgeries or, you know, you don't want to go into the chest too many times if you don't have to, so it can be nice to coordinate other surgeries so you have to think about the timing of need for possible aortic root replacement either with a valve sparing, root replacement meaning valve sparing of the aortic valve or for a more classical Bentall procedure with a, with a prosthetic valve. You have to think about now in the age of minimally invasive mitral repair, which Duke will talk a little bit more about.

You do have to think about the difficulty of those procedures with chest wall deformities or more complicated mitral valve repairs.

And finally just to reiterate on given the the strong trend and improved quality of life for people who can get a valve sparing aortic root replacement the goal clearly would also be for a surgical repair of the mitral valve if at all possible.
