This was a study, which actually was done by one of our imagers at the University of Michigan, Nick Burris and one of the things that we've been trying to understand is if a person has an aortopathy, where does the aorta grow, what do we have to be watching, what areas at greatest risk, what areas might not be so vulnerable and Nick did a very interesting study using a very sophisticated type of four-dimensional flow using magnetic resonance imaging and about 70 patients with various types of inherited aortopathies and what he discovered was actually quite unique. 

 

こちらの研究はミシガン大学放射線科のBurris医師らによるものです。大動脈疾患ではどの部位が拡張するのか、どの部位に注目すべきなのか、最も高リスクなのはどの部位か、それほど問題ないと考えられる部位はどこか、といったことが、我々が理解したいことの一つです。Burris医師は、MRIを利用した最新式の4次元フローによる、非常に興味深い研究をおこないました。様々なタイプの遺伝性大動脈疾患をお持ちの患者さん約70名を対象とし、非常にユニークな結果が得られました。

 

There were some patients who had their ascending aorta replaced and the aorta just didn't grow at all after that, the downstream aorta just stayed the same and everything was fine. There was another group where just beyond the Dacron graft and the ascending aorta, that's where the aorta tended to grow, and then there was a third group where it looked like beyond the graft the entire aorta tended to enlarge. 

 

結果は、上行大動脈の置換後、下流の大動脈にも拡張がみられず、順調な経過をたどるグループ、人工血管で修復した上行大動脈の近接部位に拡張が見られたグループ、修復後も大動脈全体に拡張がみられたグループに分かれました。

 

I illustrate this not to suggest that this is the final answer in terms of how aortas behave but it illustrates the progress we're making with imaging. I'll talk in a few minutes about the progress we're making with genetics and I believe the future is going to be far more personalized medical care where the specific genetic problem coupled with a family history, coupled with very sophisticated imaging is really going to help us plan our therapeutics and also plan appropriate imaging surveillance to maximize benefit for our patients. 

 

この研究を紹介したのは、大動脈の挙動についての最終的な回答を伝えるためではなく、画像検査がどれだけ進歩しているのかを知っていただくためです。遺伝学における進歩についてはこれからご紹介しますが、今後、より個人に最適化された医療が提供されることになると信じています。この最適化医療では、特定の遺伝子変異が、家族歴や最先端の画像検査と組み合わさることで、治療法が決定されると同時に、患者さんのメリットを最大化すべく、画像検査によって適切な管理がなされることになります。

 

So this frontier in imaging is is right in front of us. 4D flow imaging right now is a research tool but in a few years it will be common and it will, I think, vastly affect how we care for patients and, I think, in a very positive manner. 

 

最先端の画像検査はまさに目前にあります。4Dフローによる画像検査は今のところ研究のためのツールでしかありませんが、数年後には一般に使われるツールになると思われます。そして、患者さんの治療にあたり、非常に前向きに大きな影響を与えると思っています。
