I will continue this webinar with the next topic, "the follow-up during pregnancy," so in the coming minutes I will discuss when we should see our patients and who actually should see them, discuss some aspects on echo and MRI during pregnancy and I will, yeah, conclude with a topic we actually prefer to avoid, of course, during pregnancy and this has discuss some aspects regarding surgery.

So as has been addressed by the previous speakers we know that pregnancy aortic pathology has an increased risk for complications and it should be managed in expert centers, so it is very important that these cases are managed by a multi-disciplinary team, so we need at last a high risk obstetrician and anesthesiologist and also a cardiologist with specific knowledge and expertise, so if necessary as can be seen on the figure on the right we can actually extend this team with all kind of other specialists like the ICU team or geneticist and so on but because of this great variety in pathology as has already been addressed by the other speakers really individualized approach is mandatory and topics that must be addressed are both preconception counseling but, of course, also the management during pregnancy and delivery and of course and this is very important. Delivery of high risk pregnancies should be done in hospitals with the availability of cardiothoracic surgery.
これまでお話しいただいた先生方からもあったように、妊娠により大動脈の合併症リスクは高くなるため、専門医のいる医療施設での管理が必要です。そして、複数の診療科からなるチームにより管理されることが非常に大切です。チームには、ハイリスク出産を扱う産科医と麻酔医、特定の知識と専門知識をもつ循環器内科医が少なくとも必要です。スライドの右側にチームの図がありますが、必要に応じて、それ以外の診療科、例えば、ICUのチームや遺伝専門医などを加えることができます。これまでの先生のお話しからもわかるように、病態は多種多様ですので、個々の患者さんに合わせたアプローチが必須です。妊娠前カウンセリングの他に、妊娠中・出産中の管理も必要になります。そして、ハイリスク出産は、心血管手術のできる病院で行う必要があります。
