When we think about conditions associated with dissection in an abnormal aortic wall, first we characterize hereditary aneurysm disorders, some of which have outward recognizable features. We call these syndromic conditions, like Marfan syndrome, Loeys-Dietz syndrome, and vascular Ehlers-Danlos syndrome with recognizable genetic mutations.

Other conditions and even more common are non-syndromic hereditary aneurysm disorders so these run in families and are recognized to be due to several genes but don't have typical recognizable outward features of a syndrome.

Let's start with Marfan syndrome as a typical syndrome feature that you might see in the emergency room in a suspect with concern for dissection. It's important to recognize the features so you might think about dissection in these conditions so I'll spend a few slides discussing each of the common syndromic features so you can help recognize them when they present in the emergency room.

Marfan syndrome is abnormal fibrillin with an incidence of one in five thousand. People often due to a spontaneous change but it can run in a family where family members have it before the person you're seeing and we make the diagnosis based on multiple features, the family history, mutation analysis and clinical features such as very tall stature, elongated arms, and legs hyperflexibility, eye findings, things like that. 

マルファン症候群では、フィブリリン1に異常が生じており、発症率は5,000人に1人です。突然変異による発症が多いのですが、実際は遺伝性、つまり患者さんの前の世代からマルファン症候群を引き継ぐということです。診断は、複数の特徴や家族歴、遺伝子変異や、非常に長身、腕が長い、脚の柔軟性が高い、眼の所見などの臨床上の特徴に基いて行われます。

 

So, many times it's a skeletal feature to recognize. These individuals are tall and thin with pectus deformities and scoliosis and can have hyper-flexibility with the recognizable features as shown here. 

マルファン症候群が判明するきっかけの多くは骨格的な特徴です。長身でやせ型、胸骨の異常な変形や側弯症、そして、このスライドにあるような過度の柔軟性があります。

 

The person with Marfan syndrome often has an elongated face, down sloping eyes, small chin, flat cheekbones and a very tall palate with crowded teeth and have small oral pharynx. 

マルファン症候群の患者さんは面長で垂れ目、顎が小さく、平坦な頬骨で、口蓋が非常に高く、歯は叢生、咽頭口部が小さいという特徴が多くみられます。

 

Marfan syndrome, of course, leads to aneurysms of the aortic root and that's what puts them at risk for ascending aortic dissection but also at risk for descending dissection. A majority have mitral valve prolapse. 

当然ながら、マルファン症候群では大動脈基部に動脈瘤が出来やすいことから、上行大動脈のみならず、下行大動脈で解離を生じるリスクも高くなります。また、大部分の方で、僧帽弁逸脱がみられます。
