We're now understanding exactly why medications like losartan work in these conditions. Again by work that was done by Elena McFarlane we've learned that cells within the aortic root, so-called second heart field-derived cells, which are colored in orange, are sensing the effect of the Loeys-Dietz syndrome mutations and in response to that, sense to abnormality they're producing lots of TGF beta, which is shown by these little purple star bursts, that go on that goes on to stimulate neighboring cells that are colored in green, which derive from a different embryonic origin called the cardiac neural crest.

What we learned is that the excessive production of TGF beta by the second heart field cells by these orange cells is completely dependent upon the, the molecule, the receptor that is blocked by losartan, so if you treat these cells with losartan and they no longer have the ability to produce too much TGF beta and in, in mouse models that's the mechanism for prevention of aneurysms to form.

So now I think we feel more confident going into the next round of guidelines to say that drugs such as angiotensin receptor blockers: losartan, irbesartan or other family members hold strong promise in the treatment of Loeys-Dietz syndrome. We would also advocate the potential additional use of beta blockers such as atenolol or propranolol, so that will be part of the next updated guidelines.
