When should one have aortic surgery? In general, in Marfan and related disorders, once the aorta achieves 5 cm, it's recommended to have prophylactic surgery.

And this data comes from a large experience and this is one such datasets from Guillaume Jondeau from Paris looking at events in 732 patients with Marfan syndrome.

So up here says "Aortic event without surgery," and you can see when their aortas under 5 cm or 49 mm here, there were two events out of 157 patients and 675 years of follow-up. So the annual risk of an aortic dissection if the aorta is under 5 cm is 0.3, so it's not 0 but it's close to 0, 0.3.
