Pregnancy Concerns in Marfan Syndrome and Loeys-Dietz Syndrome

Okay, so talking about anesthesia. So first I just want to say good anesthesia is key for safety if you have Marfan or Loeys-Dietz syndrome and I know it's sort of some people are like, "why, why do I have to get this?"

There's two reasons. So one is we really want you to avoid pain and labor because what happens if you're in pain and breathing through contractions is that you're going to have increases in your blood pressure, increases in your stress hormones, increases in your heart rate. All of those things are putting more stress on the blood vessels so that's one reason we want to talk about an epidural.

The second reason is we want to decrease the amount of time you're pushing and labor because that valsalva pushing to have a baby could put some stress on the blood vessels as well and so we can use forceps or the vacuum to help with the delivery and that's going to be a lot easier and more comfortable if we have good anesthesia.

So, you know, those are the reasons why I think epidural is really a matter of safety for moms if you have Marfan or Loeys-Dietz syndrome.

So things that are important for this ahead of time is having spinal imaging because some women have scoliosis, some people have rods in their back.

There's something called dural ectasia, which you can see here in the bottom right, so there's these outpouching of the dura, which in cases your spinal cord, this is not necessarily a contraindication to doing regional anesthesia or an epidural. What happens though is that sometimes the medicine can pool in those little pockets and it doesn't work as well.

And there's debate among anesthesiologists of whether they would perform regional anesthesia if you have dural ectasia but I think for a majority of the people that are taking care of a lot of women with these connective tissue issues and pregnancy, we do think that it is safe with dural ectasia to attempt regional anesthesia.

The other thing if you have Loeys-Dietz is making sure that they realize that some women can have cervical spine instability and this is important if you are going to be intubated for any reason or needing to go to sleep during the delivery, that we want to make sure that, that is known ahead of time.
