The elephant trunk procedure, this is, this is important for our connective tissue disorder patients, especially Marfan’s and Loeys-Dietzs where they, they have risk for intervention on the remaining segments of their aorta and if you do a root, total arch the descending is at risk and so if you leave an elephant trunk behind you're setting yourself up for success, so let's walk through and see what that really means.

So we're taking a dacron graft. We're going to ??? upon itself so fold it in kind of like what you do for your socks, put it in and then once it's in while we have the patient under circulatory arrest we can drop that into the descending thoracic aorta like an elephant trunk hanging down. We can then unfurl the remaining graft and then reconnect the arch.

Obviously with our connective tissue disorder patients we're going to do, you know, the branch graft so that we get rid of all that bad aorta but the purpose of this is we're leaving graft down here in the descending thoracic aorta.

So in this representation this patient has an aneurysm of the descending thoracic aorta. It's not big enough to get intervened on now but it may be in the future if it grows.

So we're making it safer for the patient saving it for the surgeon and making it a shorter operative time by leaving this down there because now when we come in through the side for thoraco we'll be able to put a cross clamp down here rather than up in the arch and, you know, putting nerves at risk or blood vessels or anastomosis at risk so it's essentially a preparatory step for a stage procedure.
