So acute aortic syndromes involve various portions of the aortic wall. There's essentially three major sections of the wall. The adventitia, which is on the outside, the media, which is on the middle and the intima, which is on the inside and you see the vasa vasorum. That's blood vessels that are in the adventitia that feed the aortic wall itself and give it its nutrients.

Let's look at an aortic dissection, an aortic dissection and you can see, in number one you get an entry tear. There's a tear that occurs in the wall of the aorta and then what happens is blood shoots into that tear and creates what you can see in number two, is a false lumen. The FL is false lumen and that's where the wall splits, the intima and adventitia split apart as the media fills with blood and then eventually you can see an exit point at the top where the blood exits out and the false lumen stops.

There's a couple other things that come up. Intramural hematoma that's essentially a dissection that doesn't really have an identifiable entry or exit and some people theorize that's from bleeding from the vasa vasorum I mentioned earlier.
